Gesundheitssysteme stehen vor der Herausforderung, bei knappen Ressourcen Patientenversorgung so zu erbringen, dass sie sich an den Bedarfen und Präferenzen der Patienten orientiert. Verschiedene Studien zeigen, dass die Gesundheitsversorgung jedoch teilweise vom Angebot gesteuert wird und die Patientenzentrierung teilweise auf der Strecke bleibt. Aus diesem Grund zielt Value-based Healthcare (wertorientierte Versorgung) auf die Umgestaltung von Versorgungsprozessen und -strukturen hin zu mehr Patientenzentrierung bei gleichzeitigem Ressourcenbewusstsein ab. Das Forschungsprojekt OrgValue ist eines von drei Teilprojektes des Verbundprojektes Cologne Care Research and Development Network (CoRe-Net). OrgValue verbindet die beiden weiteren Unterprojekte LYOL-C und MenDis-CHD. In einer Analyse der an der Versorgung von Patienten aus beiden Unterprojekten beteiligten Organisationen der Gesundheits- und Sozialversorgung in der Stadt Köln wird die Umsetzung des Konzeptes von Value-based Healthcare mit Mixed Methods untersucht. Mittels qualitativen Einzelinterviews und einer quantitativen Befragung der Entscheidungsträger Kölner Versorgungsorganisationen werden der Implementierungsstand einer patientenzentrierten und ressourcenorientierten Versorgung und entsprechende Förderfaktoren und Barrieren der Umsetzung untersucht. In Fokusgruppen und Einzelinterviews mit Patienten wird zudem untersucht, was diese unter Patientenzentrierung verstehen und welche Präferenzen und Bedarfe für sie zentral sind. Dies soll einen Abgleich zwischen der Perspektive von Patienten und Organisationen hinsichtlich der Patientenzentrierung ermöglichen. Die Befragungsinstrumente decken die drei Kategorien Patientenzentrierung, Ressourcenorientierung und Determinanten der Implementierung, z.B. Klima und Kultur, ab. Zusätzlich ist eine Entwicklung von Diagnostik- und Feedback-Instrumenten zur Selbstevaluation der beteiligten Organisationen vorgesehen.

Description

Health care systems are under pressure to organize care around patients’ needs with strained resources. Several studies reveal that care is primarily supply-driven rather than patient-centered. Therefore, value-based health and social care aims to redesign care delivery around patient-centeredness whilst taken the resources spent into consideration. OrgValue is one of three scientific projects of the Cologne Care Research and Development Network (CoRe-Net). The organizations in Cologne caring for vulnerable patient groups studied in the subprojects LYOL-C and MenDis-CHD are examined in OrgValue using a mixed methods approach. The implementation status of patient-centeredness and resource orientation as well as its facilitators and barriers are assessed through face-to-face interviews with decision makers from HSCOs in Cologne. Additionally, patients’ understanding of patient-centeredness and their preferences and needs are surveyed conducting focus groups and face-to-face interviews. This opens up a comparison of organizations’ and patients’ perspectives on patient-centeredness. The qualitative results provide the basis for a quantitative survey with decision makers from all HSCOs in Cologne. All examinations include questions on patient-centeredness, resource orientation and determinants of implementation like organizational climate and culture. The development of a diagnostic and feedback-tool for self-evaluation out of the aforementioned survey is taken into consideration.
